Calvary Silver Circle is delighted to announce that Home and Community Care (HACC) services are now being offered in the Flinders Far North covering the areas of Port Augusta, Flinders Ranges, Mt Remarkable, Northern areas Mt Pirie Districts, Whyalla, Port Lincoln, Tumby Bay, Peterborough & Orroroo-Carrieton.
The HACC program is designed to help older people live independently by providing services that support their daily activities at home and in the community.
The HACC program provides the practical assistance that many older people need for independent living with services including:
- transport (to appointments and shops)
- domestic assistance (cleaning, laundry, ironing and meal preparation)
- social support (companionship)
- centre based day care (social activities in a community based setting)
We have respite care options which are designed to be flexible and responsive to meet the needs of carers and the person for whom they are caring.
We also have Level 2 Home Care Packages which is a coordinated package of services tailored to meet your care needs. The services can support you to stay at home for as long as possible, as well as give you choice and flexibility in the way that care and support is provided.
Home Care Packages are available on a Consumer Directed Care (CDC) basis.
CDC gives you the opportunity to choose the level of involvement you would like to have about the types of care and services you receive, how care is delivered and who delivers it. You will be provided with a personalised budget to see how much funding is available for services and how much money has been spent. Services offered can be:
- personal care
- domestic home care
- social support / companionship
- respite care
- case management
- home maintenance & modifications
- 24 hour personal emergency monitoring
- clinical care
