Hunter Care Group is a Mental Health organisation which provides support services in the Newcastle, Lake Macquarie, Hunter Valley, Central Coast and Mid-North Coast areas.

Our Services:
Hunter Care Group provides 1 on 1 supports for members of our community with a mental health disability and a funded NDIS Package.

This funding can be utilised for a support worker to assist in you in a range of areas such as;

-Assistance with Daily Self-Care
-Assistance to Access Community
-Support developing Independent Living Skills
-Developmental Life Skills
-Life-Stage Transition Assistance
-Education / Vocational skills development
-Travel Training
-Budgeting and Financial Skills Development
-Daily Planning Assistance
-Support attending Medical Appointments
-Capacity Building Training
-Shopping / Budgeting Support
-Meal Planning and Preparation

Coordination of Supports:
Coordination of Supports (COS) focuses on supporting a participant with engaging with services such as informal, mainstream, community and other streams of funded supports within their current NDIS Package.
We will support you in:

-Assessing different support services and what will work best for you

-Navigate you around what supports are funded in your NDIS plan and how we can implement them together

-Manage your NDIS plan (budget) for you and advise your linked in supports of the breakdown of funds available.

-Linking you in with specific supports that are funded in your plan (Clinical, Housing, Medical, Vocational)

Hunter Care Group Coordinators of Support (COS) work in our communities innovatively and efficiently with participants in how they utilise their NDIS Plan.
Communication is our focus, our management structure is designed in a way that your COS will be contactable daily.
