Since 2018 our mission is to empower individuals with disabilities to lead independent lives. Our goal is to prevent unnecessary institutionalisation by offering support and services that facilitate active community participation. As prospective members, count on us to go above and beyond in helping you achieve your goals. Our experienced NDIS support team is proficient in English, Dari, Farsi, and Hindi as well as Auslan sign. We offer a wide range of services and support under the NDIS. Optimum Care Group is based in the South East of Melbourne with NDIS carers and support coordinators serving clients throughout Melbourne, Victoria.
