The District Nurses is a Home Care Package provider, provides Home and Community Care (HACC) to under 65yrs and Community Home Support Programme (CHSP) to over 65yrs and is a registered NDIS provider. Services include Community Nursing and Community Care, support at home statewide. Care to private clients, brokered services, and clients at end of life is provided statewide.
